Determination of Needs Process Concerns or Complaints Form

If you have already attempted to resolve your issue about the administration of the determination of needs process with your care coordination team, please use this form to submit your concern or complaint.

The AccessOAP independent review office conducts procedural reviews of the determination of needs process to determine if all steps related to the administration of the determination of needs have been followed correctly.

By submitting this form, you understand that the independent review office may review documents, gather evidence, conduct interviews, and consider the input of all parties involved before reaching a decision on the matter being reviewed. If procedural errors are discovered, families will be provided with options for how they can be rectified.

If you remain unsatisfied with the resolution presented by the independent review office, you will have the option to submit your concerns or complaints to a third-party for review. The role of the third-party is to review decisions made by AccessOAP’s independent review office. Based on the outcome of their review, the third-party can uphold a decision or advise AccessOAP to re-administer part or all of the determination of needs interview.
